Community Hospice & Palliative Care improves end-of-life care by providing the highest quality, patient- and family-centered support to individuals facing serious illness. Serving residents in Baker, Clay, Duval, Putnam, Nassau, and St. Johns counties, as well as 11 additional counties in north-central Florida, the organization offers services to meet the needs of patients and their families.
Offering Hospice Care, Advance Care Planning, Palliative Care, and Veterans Resources
The Hospice Care Program assists individuals who have been diagnosed with a life expectancy of 12 months or less. For Medicare recipients, eligibility begins with a prognosis of six months or fewer. This program ensures that patients are treated with dignity throughout their journey, with a focus on choice and personalized care. Advance Care Planning (ACP) is a service that empowers individuals to make informed decisions about their future healthcare, particularly if they are unable to speak for themselves. Through ACP, individuals can clearly document their medical care preferences, appoint a healthcare surrogate, and prepare legally recognized forms such as advance directives and living wills.
Community Hospice & Palliative Care provides free guidance throughout the entire ACP process, addressing key questions and facilitating discussions with doctors, caregivers, and loved ones. By planning ahead, individuals gain peace of mind knowing their final wishes will be understood and respected, while also easing the emotional and decision-making burden on their families.
The Palliative Care Program supports individuals at any stage of a serious illness by managing pain, reducing stress, and improving quality of life. This care is delivered by Advanced Practice Registered Nurses (APRNs) in collaboration with the patient's primary physician. The program is personalized according to the patient's goals and the patient's physical and emotional well-being throughout treatment. Eligibility may include a life-limiting illness with a prognosis of less than two years, declining health, or the need for advance care planning.
Since 1979, the Veteran Care Program has honored and supported military service members and their families. Recognizing the unique emotional and physical challenges veterans face, this program offers comprehensive end-of-life services, coordinated by a dedicated team of physicians, nurses, and social workers. Veterans also have access to free resources such as benefits guides, burial kits, and ongoing emotional support for their survivors.
Providing Emotional Support, Perinatal Hospice, and Caregiver Support Through the Community PedsCare Program
The Community PedsCare Program provides compassionate hospice and palliative care for infants, children, adolescents, and young adults up to age 21 who are living with a life-limiting or complex condition. Families receive coordinated care and emotional support from a dedicated pediatric team that understands the challenges of caring for a seriously ill child. Services include pain management, medication assistance, nursing support, counseling, grief support, spiritual and cultural care, and help with transportation, meals, and insurance navigation.
For parents facing a prenatal diagnosis of a life-limiting condition, the Perinatal Hospice Care Program provides medical guidance, birth planning, emotional support, and bereavement services. Additionally, Community Hospice acknowledges the role caregivers play and offers Support and Enhancement Care to help them recharge and find peace of mind. These services can take place in the home or within a facility, providing a space for caregivers to rest, reflect, or receive emotional support from nursing staff.
Short-term Respite Care is available in-home to provide temporary relief from full-time caregiving responsibilities.
