Patient-Centered Care Tailored to a Patient's Physical and Emotional Needs

Haven Hospice is a service-driven organization dedicated to supporting patients and families through the complexities of end-of-life care. Specific services include advanced care planning, palliative care, hospice care, specialized dementia care, grief and bereavement support, and dedicated caregiver support programs. With locations in Chiefland, DeLand, Gainesville, Jacksonville, Lake City, Orange Park, Palatka, and St. Augustine, the organization ensures that communities across the region have access to reliable and supportive care.

Advance Care Planning and Palliative Care Consultations

 An interdisciplinary team delivers each service focused on enhancing quality of life, alleviating pain and discomfort, and providing guidance to both patients and their loved ones. Through the advance care planning program, trained specialists work closely with patients to help them clearly define and document their healthcare goals, values, and preferences.

Palliative care consultations, typically covered by Medicare, Medicaid, and most private insurance plans,  provide patients with an added layer of support while managing serious or chronic illness. These consultations center on meaningful, in-depth conversations between the patient, their loved ones, and a specialized care team to address pain management, symptom relief, and overall treatment goals. 

The primary aim is to improve quality of life by aligning medical care with the patient’s personal values, preferences, and needs. During a palliative care consultation, specially trained clinicians work collaboratively with the patient’s existing healthcare providers to ensure a well-coordinated approach to care. They communicate directly with physicians, nurses, and other specialists, and engage in open discussions with the patient and their family. 

This team-based approach helps clarify complex medical information, explore available treatment options, and support decision-making. In addition to managing physical symptoms such as pain, fatigue, or shortness of breath, palliative care specialists also address emotional, social, and spiritual concerns. Dementia Care and Bereavement Support Groups

Haven’s dementia care program provides patients and their families with a personalized care plan supported by an interdisciplinary team. Services are coordinated across a variety of settings, including assisted living communities, group homes, and other residential care environments. Haven Hospice offers free bereavement support groups that are open to all members of the community, providing a compassionate space for individuals to share their experiences and process grief alongside others who understand their journey. Bereavement services are available across 18 counties, including the St. Augustine area.

Youth Camps and Seminars to Support Caregivers

In addition, Camp Safe Haven is available to youth ages 5 to 18, offering a supportive environment where young individuals can process grief. Through therapeutic and engaging activities, participants are encouraged to express their emotions, build coping skills, and connect with peers who are experiencing similar losses.
