Helping Clients Navigate the Healthcare System
Since its founding in 1998, the Northeast Florida AIDS Network (NFAN) has served as a trusted resource and support system for individuals living with HIV or AIDS throughout Northeast Florida. NFAN’s comprehensive Medical Case Management program helps clients navigate the often-complex healthcare system and connects them with specialized providers who understand their unique needs.
Through this program, clients gain access to ongoing medical care, life-saving medications, dental treatment, and vital mental health services.
Connecting Individuals with Community Services
The role of NFAN’s case managers extends well beyond medical support. They work closely with clients to address the day-to-day challenges that can stand in the way of treatment and stability. This includes providing co-pay assistance to reduce financial strain, coordinating reliable transportation so clients can attend essential medical appointments, and connecting individuals with additional community resources that promote wellness and security.
