
Facilitating Access to Core Healthcare Services
The Medical Program offered by St. Johns County Social Services assists uninsured individuals who are either actively employed or living with a disability and coping with progressive or life-limiting illnesses. Eligible participants may receive coverage for inpatient physician services, including hospital-based care. The program also offers access to specialty physician services for diagnosing and managing advanced or complex medical conditions.
Beyond inpatient and specialist care, the program extends coverage to various outpatient services. These include physical therapy, diagnostic imaging such as X-rays and scans, and qualified home health care services.
Please note that the St. Johns County Medical Program does not cover all medical conditions or circumstances. For additional information about covered services, it is strongly recommended that you contact the department directly.
Eligibility Requirements
To qualify for the Medical Program, applicants must be residents of St. Johns County and provide valid identification or proof of residency. Acceptable documents include a lease agreement, a utility bill, or a voter registration record.
In addition to residency documentation, applicants are required to submit a physical referral or hospital discharge papers. Social Security cards must be provided for all household members. Proof of employment is also necessary for the applicant and all household members. It may include recent pay stubs, a letter of financial support indicating the dollar amount provided, or documentation verifying employment, self-employment, or unemployment status.
Proof of Medicaid eligibility is required for parents caring for children. Furthermore, all household members must submit current bank statements as part of the application process.
